Our commitment to your privacy

Cary Vision Care is committed to protecting the privacy of your personal health information. We are required by law to maintain the privacy of protected health information, provide you with this notice of our legal duties and privacy practices, and abide by the terms of the notice currently in effect.

How we may use and disclose your health information

We may use and disclose your protected health information for treatment, payment, and health care operations. This includes sharing information with other health care providers who are treating you, obtaining payment from your insurance company for services provided, and internal operations such as quality assessment, staff training, and business planning.

Other uses and disclosures

Complaints

If you believe your privacy rights have been violated, you may file a complaint with our practice or with the U.S. Department of Health and Human Services. We will not retaliate against you for filing a complaint.
